Whether the current care arrangements have been in place for some time or are the result of the support I have provided, a regular review is recommended to ensure that care needs continue to be met and to provide an opportunity to discuss any changes or concerns.
Regular visits and ongoing telephone support following either Your Consultation or my Independent Assessment of Health and Care Needs. This service is supported by a letter summarising any concerns or required changes and my suggested next steps including recommended referrals to and /or liaison with other professionals where necessary.
A phone call will follow to answer any queries you may have and to provide an opportunity to discuss next steps.
